Addendum to Teaching Contract

Teacher and district agree that the contract of teacher shall be modified for the semester(s) commencing_____________________, 20___, and ending __________________, 20___, to afford the teacher extended leave pursuant to Board Policy GCCAG, as in effect at the time of execution of this addendum.

The purpose of the extended leave of absence is:______________________________ as detailed on Exhibit A, attached hereto and incorporated herein by this reference.

In consideration of the granting of extended leave by the district, teacher has read, understands and accepts the provisions of Board Policy GCCAG, including, but not limited to, those provisions concerning contributions for insurance coverage and PERA, non-accrual of sick leave, failure to utilize the leave for its designated purpose, and cancellation of teacher's contract with the district in the event teacher fails to utilize the leave for its designated purpose (see Exhibit A) or fails to comply with the notification and reporting provisions of Board Policy GCCAG.
