Garfield Re-2 Policies

JLCD-E 2-Administering Meds

JLCD-E 2

Dear Parent:

In case you were unaware of the school's policy on giving of medicine to students by school personnel, I would like to bring you up to date on this.  If your child must have medication of any type, including over-the-counter medicine, given during school hours, you have a few choices:

1) You may come to the school and give it to the child at the appropriate time.

2) You may get a medication form from the school and have your doctor indicate on the form the drug, dose, time to be given, and be sure he/she signs the form.  We also need a pharmacy-labeled bottle containing the medication and instructions to be given.

3) You may discuss with your doctor an alternative schedule of medication so it can be given outside of school hours.

I am enclosing a copy of the medication form.  Please feel free to make multiple copies of this form so you will always have one for the doctor.

I feel in fairness to those giving the medications and in safety to your child, these policies must be followed strictly.  We ask this not to make things difficult for you, but to ensure the health and well being of all students.

Remember, the only way we can give medication at school is with a school medication form filled out by the doctor and signed by him/her with a proper container.

Thank you for your cooperation.
